Weight management Support
The National Exercise Referral Scheme (NERS) is a Welsh Government funded scheme which has been operating since 2007. It is the only national exercise referral programme in the UK and it aims to standardise exercise referral opportunities across all Local Authority and Health Board areas in Wales.
NERS is an evidence-based health intervention incorporating physical activity and behavioral change techniques to support referred clients to make lifestyle changes to improve their health and wellbeing.
The Welsh Local Government Association previously led the operational management of the national NERS programme, with Public Health Wales undertaking strategic oversight. From the 1st April 2022, Public Health Wales now oversees both operational and strategic management. Public Health Wales also manages the grants to 22 Local Authorities, Leisure Trusts and one Health Board which are responsible for the local delivery of the scheme.
Aims of NERS
- To engage those who are sedentary and at risk of or with established disease to participate in regular physical activity
- To increase levels of confidence in engaging in regular physical activity
- To contribute to reduced risk of disease progression or recurrence of acute events
- To improve mental wellbeing and motivation for self-care.
What does NERS provide?
NERS provides access to tailored and supervised exercise for those aged 16+ who are inactive and at risk of, or currently experiencing, a long term or chronic health condition.
Individuals can be referred by health professionals to a local NERS scheme and will receive an initial assessment, a 4 week review and a suitably graded exercise programme for 16 weeks. At the end of the programme, participants are encouraged to progress to a variety of opportunities within the local community to continue to be physically active. Participants are also followed up at 52 weeks (1 year) to assess if they remain physically active.

If you require further information about a local scheme, such as where it is delivered, where to send a referral form or how to get referred, then please contact your local NERS Coordinator.

SLIMMING WORLD (ONLY FOR PATIENTS THAT HAVE NEVER ATTENDED SLIMMING WORLD BEFORE)
Slimming World on Referral gives GPs, nurses and other health professionals the opportunity to offer free membership of a local Slimming World group to those patients who they feel would benefit. Referred patients attend at no cost to themselves for an initial agreed period (in multiples of 12 weeks blocks); the cost to the commissioner is subsidised by Slimming World, where all weight loss resources are provided.
How does it work?
Patients are provided with a referral form by a health professional (typically a GP, nurse, midwife or Health Trainer) and these are redeemed at a local Slimming World group. The patient is able to choose the most convenient Slimming World group to attend from a list of local groups.
